It pains me to watch somebody I care about struggling, floundering, and neither asking for nor accepting help. It pains me more to watch him/her drag parts of his/her family down along side.

I have been rereading The Wizard's Dilemma by Diane Duane (semi-spoiler ahead), in which Nita works so hard to avoid telling those who care about her and could help her what is going on - and repeats this again and again through the story.

She gets better at it fairly quickly. Unfortunately, my friend seems not to be learning so swiftly. OTOH, he is a bit older than she and has learned not to rely on outsiders for a long long time...
